You can't use the Tome either right or wrong.

when your Con is 15 you get one extra HP per level of the PC

when your Con is 16 you get two extra HP per level of the PC

when your Con is 17 you get three extra HP per level of the PC

when your Con is 18 you get four extra HP per level of the PC

The two last counts only if you are a ranger paladin or fighter. So nomatter when you use the tome of constitution it will just adjust your HP according to the list. You cant mess up :) And you are completely right you went from 17 in constitution to 18 in constitution. So you go from 13 HP to 14 HP per level. :)

Indeed, the Constitution modifier for fighters, rangers and paladins is one higher when your constitution is raised from 16 to 17 or 17 to 18.

You rolled very well, 70 Hit Points already, that ain't bad!
