I bet you're aware of certain weapon trainers. I'm not sure which one can't provide you all five proficiency slots filled, but I know for certain that Korur (Lower Ward) can.

But to get it, you must have both fighter specialization bonuses!
These bonuses you get ONLY if you reach level 7 and level 12 first in fighter class.

If you've reached level 7 or level 12 first in mage or thief class, you get specialization bonus for mage or thief respectively, and your chance to get mastery (in any weapon) is doomed.

    [EDIT]
Oh, yes, you must have enough "unused" proficiency slots to be filled by the trainer. You get those by levelling up. If you don't have them enough, go to UnderSigil or Rubikon - those two are in fact arenas with nice experience to collect. ;)

Rubikon is in other words - Modron Maze.

Go to Curiosity Shoppe and buy a toy selectable from special items. Talk to Yvanne in Art and Curio galeria or to Modrons in Brothel about that toy to learn what's so special about it.
In fact, you'll learn that the toy is a instant portal key.
