Cayron's Scar
*Evil* Chief Red-Maw
I show this part from the perspective of the evil party. I start off at #1 on the map of Cayron's Scar. There will be a large group of Lagufaeth led by Chief Red-Maw at #5, but the battle isn't difficult.
Chief Red-Maw leaves behind a ring called the Iron Circle. Zakara wears it for the rest of the game. Her fighting style is heavily dependent on Damage Reduction, and not so much on Deflection. So it makes sense for her to get an item that gives her a hefty +4 bonus to Constitution and another boost to Damage Reduction should she sustain enough damage in a battle. The -5% Attack Speed is negligible in this instance, since it will easily be overborne by the bonuses to Attack Speed provided by her two-weapon style.
*Note* It is possible to take a shortcut through the ice floes at #13. There are a few options to get across safely to the other side without any injuries based on enlarging the floes themselves temporarily:
Snow Bears
I easily kill off the Snow Bears at #12.
Ondrite Fanatics
The group of Ondrite Fanatics at #7 can also be defeated without much trouble through standard tactics.
Master Diometto
A large group of Ondrite Fanatics will be at #9, and led by a Monk named Master Diometto. I have to put a little more effort into this one. Hiravias uses Moonwell, Relentless Storm and Venombloom to give his comrades the decided advantage. I have Ituumak race forward at the outset and before battle positions settle, so that he and Sagani can go to work on the distant spellcasters. Grieving Mother of course makes use of Amplified Wave whenever she can.
Master Diometto leaves behind an amulet called the Lost Periapt of the Winding Path, but I'll just sell it.
Ondrites and Lagufaeth
There will be several both of Ondrite Fanatics and Lagufaeth at #15. I defeat them through the usual tactics.
Eyeless
Now I go around the southwest corner taking out the odd Eyeless that I come across towards the southwest corner. Individually they're easy enough, especially when Hiravias uses his Moonwell and Alliria uses both her Flames of Devotion. Grieving Mother will also use Disintegrate on them whenever she has enough Focus.
Eyeless and Tentacles
There will be a couple of Eyeless and several Tentacles at #17. I have Kana switch to his Phrase modal that includes only 1st-level Chants. Next, I lure the single Eyeless just a little east of #17 away from the rest so that I can kill it piecemeal.
Now I approach #17 from the north. I go to work on the first Tentacle. The benefit of switching to the 1st-level Phrase modal is that Kana can get to 7 Chants faster, and therefore can sooner use his Ancient Instruments of Death invocation that summons several weapons. Those weapons hold up the remaining Eyeless and the two other Tentacles to the south, allowing the party a free hand to take out the second Tentacle to the left. The battle is much easier for it.
Entrance to the Fragment
There will be a tightly packed group of four Eyeless at #18. I begin fighting the two that are most to the left. Alliria puts Reinforcing Exhortation on both Kana and Devil of Caroc. Hiravias casts Moonwell and Relentless Storm.
Kana then uses Ancient Instruments of Death once he has 7 Chants so as to summon the four weapons and have them hold up the Eyeless that are a little more to the east.
After that, I have Hiravias put a Beetle Shell on Kana and Grieving Mother use Defensive Mindweb in order to make sure I can last over the long haul.
The entrance to the Ionni Brathr Fragment is at #19.
