This is the personal quest of Aloth, and begins to unfold as you near completion of the Never Far From the Queen quest.
Aloth will have a hallucination of sorts when the party reaches #15 on the Temple of Woedica map. This is one of the triggers for his quest.
He will want to speak to you again after you've completed the Never Far from the Queen quest. There will be a Stoic option when the dialogue first begins.
He lets on that, like you, he is an Awakened Soul who shares his body with somebody named Iselmyr who surfaces in a confrontational way whenever Aloth may himself be in danger. That leads to him expressing a desire to consult an expert on Awakened Souls in Defiance Bay. You can choose a Benevolent reply in response to the request. The dialogue is the next trigger for Aloth's personal quest.
If you speak to Head Warden Ethelmoer, who's at #3 on the map of the Sanitarium, he'll indicate that you should speak to Bellasege. She is downstairs.
The stairs down to the basement are at #13 on the main floor map.
The stairs take you down to #14 on the basement map. Bellasege is in the room at #15. She'll hook Aloth up to a machine and start to analyze Aloth's soul.
At some point things will get pretty intense during the experiment, and you will have Aggressive, Benevolent, Passionate and Rational dialogue options available.
Aloth will a bit later express disbelief that Iselmyr acts as a kind of 'guardian angel' who emerges only when Aloth is in danger. Diplomatic and Passionate replies become available.
Aloth will also want to confiscate Bellasege's notes. Deceptive and Stoic replies will permit him to do so. Honest and Passionate replies will dissuade him against it. There are no in-game consequences for whether or not he confiscates the notes.
