This door will not open unless you first obtain permission from Head Warden Ethelmoer on the first level to investigate the North Patient Ward in the course of The Man Who Waits quest.
This room will be filled with Patients and Flesh Constructs. One patient here, Freyol, will be able to offer information on Caedman Azo's motivations as part of The Man Who Waits quest.
You can also ask what brought her to the Sanitarium. She initially expresses reluctance, to which Clever, Honest and Passionate replies are possible.
She is then willing to reveal that she herself had an Awakening, which revealed that she was once a noblewoman living in luxury in a past life. Her current life angers her since it implies the Gods punished her by forcing her soul back in to the lottery of the Great Wheel of Souls. Diplomatic, Passionate, Rational and Stoic replies are possible when she expresses resentment.
The crate here has a Hide Armor called Blaidh Golan.
Caedman Azo is here. At some point you'll need to get his permission to access the North Ward as part of The Man Who Waits quest. When you ask him for permission, any reply besides "you had best cooperate" will suffice to get the North Ward Key. Even if you seemed to have blown it with the 'cooperate' dialogue option, you can simply speak to him again and select a different dialogue option to get the Key.
